Senior Director ACO Complex Care Management, Boston Medical Center Health System
Erin Nahrgang, RN, CCM is the Senior Director of ACO Complex Care Management for Boston Medical Center Health System. In this role, Erin leads a dynamic program serving Medicaid members with complex health and social needs as they navigate their health related goals. She has expertise in complex care, multi-disciplinary team building, advocacy and cross-sector partnerships that support the well-being of vulnerable communities and individuals. She is passionate about elevating the stories and experiences of the beneficiaries and staff at the heart complex care, and building pathways for personal and professional growth in the complex care field. Erin’s work history includes: Transitional Care Management at CareOregon, complex care for patients with heart failure at Oregon Health and Science University, as well as in an advisory capacity for ACCESS, a community based health and education center in rural Uganda.
Erin received her Bachelor’s of Nursing Degree from Oregon Health and Science University, and her Bachelor’s of Arts in Intentional Studies with a focus on healthcare delivery in low resource settings from the University of Denver.
